TRAVEL LIABILITY WAIVER AND PHOTO RELEASE

The Event or Trip (the “Event”): 2025 Arizona Cardinals NFL Draft Experience Road Trip

Date(s) of the Event: April 23 – April 26, 2025

Event Location: Green Bay, Wisconsin

IMPORTANT: THIS IS A WAIVER AND RELEASE OF POSSIBLE LEGAL CLAIMS FOR PERSONAL INJURIES AND DAMAGES. PLEASE READ THIS DOCUMENT CAREFULLY.

In consideration of the opportunity afforded and my right to participate in the Event, I, as the Participant, agree as follows:

I acknowledge that my participation in the Event could involve risk of serious bodily injury, illness, death, and/or property damage, and despite safety precautions, the Released Parties cannot guarantee safety thereof, as all risks cannot be prevented. I assume and accept all risk of bodily injury, illness, death, property damage and other harm connected with my participation in the Event. I understand that if I have any risk concerns regarding travel, I should discuss the risks associated with my participation with the Event coordinators before I sign this document and before travel begins.
I acknowledge that medical insurance is my individual responsibility, and I understand that I am responsible for ensuring that my medical insurance is valid in the Event Location.
I acknowledge that I am solely responsible for any medical, health or personal injury costs relating to my participation in the Event. Should I become ill or injured, I consent to emergency medical treatment in the event such care is required, at my own cost. I am further aware that any medical, health and personal injury costs resulting from or relating to the activities undertaken pursuant to the Event will be my sole responsibility.
I further acknowledge that if I drive my own vehicle, or am a passenger in another’s private vehicle in connection with this Event, that Released Parties cannot be responsible for assuring the safety and reliability of such private transportation or driver, nor for any non-sponsored activities and travel that I might choose to participate in before, during or after the Event, and I therefore accept the risks and responsibilities associated with such private vehicle travel and activities.
I hereby indemnify, hold harmless, covenant not to sue, and forever discharge Arizona Cardinals Football Club LLC, the National Football League, their affiliates and their respective officers, directors, employees and volunteers, subcontractors, sponsors and agents (collectively, the "Released Parties") from any and all liability for all present and future claims, demands, losses, damages and costs, including reasonable attorneys' fees, that arise out of or in connection with any personal injury, even injury resulting in death, disability, property damage, property theft, and/or other loss suffered by me (collectively, “Claims”) in connection with my participation in the Event, including any Claims arising out of the negligence of the Released Parties. This Release is also intended to include all Claims of my family, estate, heirs, personal representatives or assigns.
PHOTO RELEASE: I authorize the Released Parties to take photographs and videotapes and to record my voice, conversation and other sounds during and in connection with the Event. I agree to let the Released Parties use my name, likeness and demographic information free of charge in any manner and for any purpose in any media now known or hereafter developed.
I am not under any legal disabilities that prevent me from being legally bound by this Travel Liability Waiver and Photo Release (this “Release”). I agree that this Release, and any dispute arising from or involving this Release, shall be governed by and construed in accordance with the laws of the State of Arizona.
I certify that I am 18 years of age or older and agree to the Arizona Cardinals Football Club's Terms & Conditions and understand that personal information will be used as described in the Arizona Cardinals Football Club Privacy Policy.
I have carefully read and fully understand the meaning of this waiver and release of all claims, and that I have voluntarily signed below.
