RELEASE OF LIABILITY, WAIVER OF CLAIMS, AND ASSUMPTION OF RISKS

The Event or Trip (the “Event”): 2025 Away Game Trip - Dallas

Date of the Event: November 1–4, 2025

Event Location: Dallas, TX

IMPORTANT: THIS IS A WAIVER AND RELEASE OF POSSIBLE LEGAL CLAIMS FOR PERSONAL INJURIES AND DAMAGES. PLEASE READ THIS DOCUMENT CAREFULLY.

In consideration of Arizona Cardinals Football Club LLC and its affiliated or related persons and entities (collectively, the “Club”) permitting me, and/or (if applicable) any minor child (the “Child”) that I am the parent or legal guardian of, (collectively, referred to below as “I” or “me”) participating in the Event, I, on behalf of myself, my heirs, executors, administrators, successors, assigns, agree to the terms and conditions set forth in this release (the "Release"):

1. I acknowledge that as part of my (including my Child, if applicable) participation in the Event there are dangers, hazards, and inherent risks to which I (including my Child, if applicable) may be exposed, and despite safety precautions, the Released Parties cannot guarantee safety thereof, as all risks cannot be prevented. I assume and accept all risk of bodily injury, illness, death, property damage and other harm connected with my (including my Child, if applicable) participation in the Event. I understand that if I (including my Child, if applicable) have any risk concerns regarding travel, I should discuss these risks with the Event coordinators before I sign this document and before travel begins.

    

2. I acknowledge that medical insurance is my individual responsibility, and I understand that I am responsible for ensuring that my (including my Child, if applicable) medical insurance is valid in the Event Location.

    

3. I acknowledge that I am solely responsible for any medical, health or personal injury costs relating to my (including my Child, if applicable) participation in the Event. Should I (including my Child, if applicable) become ill or injured, I consent to emergency medical treatment in the event such care is required, at my own cost. I am further aware that any medical, health and personal injury costs resulting from or relating to the activities undertaken pursuant to the Event will be my sole responsibility.

    

4. I further acknowledge that if I (including my Child, if applicable) drives their own vehicle, or is a passenger in another’s private vehicle in connection with this Event, that Released Parties cannot be responsible for assuring the safety and reliability of such private transportation or driver, nor for any non-sponsored activities and travel that I (including my Child, if applicable) might choose to participate in before, during or after the Event, and I therefore accept the risks and responsibilities associated with such private vehicle travel and activities.

    

5. I hereby indemnify, hold harmless, covenant not to sue, and forever discharge the Club, the National Football League, each of their respective parents, their affiliates, their respective officers, directors, employees, representatives, shareholders, successors, assigns, volunteers, subcontractors, sponsors, agents, and any and all other companies or persons associated with the Event (collectively, the "Released Parties") from any and all liability for all present and future claims, demands, losses, damages and costs, including reasonable legal fees, whether arising from a claim brought by or on behalf of me (including my Child, if applicable) or any third party, that arise out of or in connection with any personal injury, even injury resulting in death, property damage, and/or other loss suffered by me (including my Child, if applicable) in connection with my (including my Child, if applicable) participation in the Event (collectively, “Claims”), to the fullest extent permitted by law, including any Claims arising out of the negligence of the Released Parties, breach of contract, or breach of any statutory or other duty of care, including under any occupiers’ liability legislation. This Release is also intended to include all Claims of my family, estate, heirs, personal representatives or assigns.

    

6. PHOTO RELEASE: I authorize the Released Parties to take photographs, videotapes and other recordings to record my (including my Child, if applicable) image, voice, conversation, sounds and other pictorial, audio and visual representations and expressions (collectively, the “Media”) during and in connection with the Event for the purposes of publication, display or broadcast (including by print, web transmission, digital display and through all other form of media) and agree that any such Media are the property of the Club. I agree to let the Released Parties use my (including my Child, if applicable) name, likeness and demographic information free of charge in any manner and for any purpose in any media now known or hereafter developed. I relinquish any present or future claim for reimbursement for any use of the Media. I hereby release the Club, its affiliates, employees, representatives and agents from any and all claims, demands, costs and liability that may arise from the use of the Media.

    

7. I am not under any legal disability that prevent me from being legally bound by this Release. I agree that this Release, and any dispute arising from or involving this Release, shall be governed by and construed in accordance with the laws of the State of Arizona, without giving effect to its conflicts of laws principles.  I agree to be subject to the sole and exclusive jurisdiction of the courts located in Arizona.

    

8. I certify that I am 18 years of age or older and agree to the Club's Terms & Conditions, as they exist from time to time, and which can be found at https://www.azcardinals.com/about-us/terms-conditions and understand that any personal information that I provide or is obtained of me in connection with or during the Event (including any Media) may be published, stored, transferred and otherwise used by the Club in accordance with the Club’s Privacy Policy, as it exists from time to time, and which can be found at https://www.azcardinals.com/about-us/privacy-policy.

I ACKNOWLEDGE THAT I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL OF THE TERMS OF THIS RELEASE, THAT I AM VOLUNTARILY SIGNING BELOW, AND THAT I AM WAIVING SUBSTANTIAL LEGAL RIGHTS (ON MY BEHALF AND ON BEHALF OF MY HEIRS, EXECUTORS, ADMINISTRATORS, AND NEXT-OF-KIN, AND/OR, WHERE APPLICABLE, MY CHILD), INCLUDING THE RIGHT TO SUE THE CLUB AND THE RELEASED PARTIES.

I hereby execute this Waiver and Release on Tuesday, September 16, 2025.
