PHOTO RELEASE, RELEASE OF LIABILITY, AND WAIVER OF CLAIMS 

The Event or Trip (the “Event”): Arizona Cardinals Kids Club Member ID

Date of the Event: Thursday, April 23, 2026

IMPORTANT: THIS IS A WAIVER AND RELEASE OF POSSIBLE LEGAL CLAIMS FOR DAMAGES. PLEASE READ THIS DOCUMENT CAREFULLY.

In consideration of Arizona Cardinals Football Club LLC and its affiliated or related persons and entities (collectively, the “Club”) permitting the minor child (the “Child”) that I am the parent or legal guardian of to participate in the Arizona Cardinals Kids Club (“Event”), I agree to the terms and conditions set forth in this release (the "Release"):

1. I hereby grant the Club and its employees, directors, officers, staff, volunteers, agents, and representatives the right and permission to photograph my child and use the photo during and in connection with the Event for the purposes of publication, display, and other lawful purposes (collectively, the “Media”). I understand and agree that no royalty, fee, or other compensation shall be payable to me, the Child, or any member of my family in connection with such use.
2. I HEREBY RELEASE AND FOREVER DISCHARGE THE CLUB AND ITS AFFILIATES, AND THEIR RESPECTIVE EMPLOYEES, OFFICERS, DIRECTORS, AGENTS, AND REPRESENTATIVES FROM ANY AND ALL CLAIMS, DEMANDS, COSTS AND LIABILITIES OF ANY KIND ARISING OUT OF OR RELATED TO THE CREATION, USE, OR DISTRIBUTION OF THE MEDIA.
3. I represent and warrant that I have the legal authority to enter into this Release. I agree that this Release, and any dispute arising from or involving this Release, shall be governed by and construed in accordance with the laws of the State of Arizona, without giving effect to its conflicts of laws principles.  I agree to be subject to the sole and exclusive jurisdiction of the courts located in Arizona.
4. This Release constitutes the entire agreement of the Club and me with respect to the subject matter contained herein and supersedes all prior and contemporaneous understandings, agreements, representations, and warranties, both written and oral, with respect to such subject matter. If any term or provision of this Agreement is held to be invalid, illegal, or unenforceable in any jurisdiction, such invalidity, illegality, or unenforceability shall not affect any other term or provision of this Release or invalidate or render unenforceable such term or provision in any other jurisdiction. This Release is binding on and shall inure to the benefit of me and my heirs and next-of-kin, and the Club and its successors and assigns.
5. I certify that I am 18 years of age or older and agree to the Club's Terms & Conditions, as they exist from time to time, and which can be found at https://www.azcardinals.com/about-us/terms-conditions and understand that any personal information that I provide or is obtained of me in connection with or during the Event (including any Media) may be published, stored, transferred and otherwise used by the Club in accordance with the Club’s Privacy Policy, as it exists from time to time, and which can be found at https://www.azcardinals.com/about-us/privacy-policy.

I ACKNOWLEDGE THAT I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL OF THE TERMS OF THIS RELEASE, THAT I AM VOLUNTARILY SIGNING BELOW, AND THAT I AM WAIVING SUBSTANTIAL LEGAL RIGHTS (ON MY BEHALF AND ON BEHALF OF MY CHILD, HEIRS, EXECUTORS, ADMINISTRATORS, AND NEXT-OF-KIN), INCLUDING THE RIGHT TO SUE THE CLUB AND THE RELEASED PARTIES.

I hereby execute this Waiver and Release on Thursday, April 23, 2026.
